Please provide first and last name, phone number, and email address on entry - Ballot must be 50% filled out
- Readers can ONLY vote for the same business 5 times. (If you vote for the same business more than 5 times, the ballot will be voided.)
- To ensure accuracy of your nomination for Best Restaurant please check spelling of company names and location
